August 15 NWB Performance Fee is $25. Performance Fee checks should be made out to Northwest Ballet or NWB, tuition checks should be made out to Northwest Ballet School.
Teen & Adult Open Summer Classes:
2 separate three week sessions are offered, please indicate your choices at the beginning of this form.
Dance Cards are available:

Registration fee is $15 and the tuition balance is due by June 15, 2008.
I, the Parent/Legal guardian understand all deposits are non-refundable should the student be accepted and cancel their reserved place in the NWB Summer Dance program. I understand that I am responsible for timely payment of tuition and agree to pay costs for collection of any unpaid tuition or fees.

I understand dance requires physical exertion. i agree to assume full responsibility for any risks, injuries or damages that might occur as a result of participating in the summer dance program.

I agree not to hold NWB (Northwest Ballet), association, any faculty, employee or volunteer liable for injuries sustained or illnesses contracted while myself or my child is a student of NWB.

I give permission for NWB to take photos of my daughter/son to use for promoting the school.
